We continue to use ATS, the on-line software program that allows students, parents, and coaches to log in check on the athletes file. For the 2021-2022 school year physical packets will only be accepted on-line, including re-certification forms, the physician page (previously Section 6 now Section 7) may be uploaded, e-mailed to me, or turned in to the black mailbox outside the athletic training room. Forms will be available online 6/1/2021.
First time users can find a YouTube tutorial here https://youtu.be/RYK0PaygKAc
Your student will be sent a copy of this e-mail to their CRSD.org e-mail address also, with their login information. Their username and password will appear at the bottom their e-mail. We no longer have a separate login for the student and parent. Both should login with the student’s CRSD student ID and password. Please click on the following link to login: http://crsd2.atsusers.com
When you log in for the first time with your assigned password you will be asked to change it. Please keep the password to under 10 characters.
The GENERAL tab is for information about the STUDENT ATHLETE. The emergency tab is where the parent information should be. ****PLEASE NOTE**** We have tied the ATS system to the information that is in the HAC system. The information is updated nightly. If you see information that is incorrect or out of date in ATS, DO NOT attempt to correct it in the ATS system, it will be overwritten during the update. To correct information please log in to the HAC system (https://hac.crsd.org/HomeAccess) and make updates there or contact Central Registration (Registration@crsd.org) to make updates.
This system allows the student to log in and request an evaluation appointment for an injury, or simply to report that they were injured. This information will allow me to get in touch with them about these things.
Please have your student login and look around the system. The default username is the student ID number; the generic password is CRN_1. If they changed it during a previous season they will need to look in their CRSD e-mail to see the current password that is attached to my e-mail. Once they are in the system they can change their password.
To fill out the physical packet click on the “Forms” tab. There are 7 forms that need to be completed and submitted. To create a new form pull down the dropdown menu on the bottom left side of the screen and select the form you want to complete. Once the form is selected click the new button and a blank form will appear.

PIAA SECTION 3: UNDERSTANDING OF RISK OF CONCUSSION AND TRAUMATIC BRAIN INJURY
PIAA SECTION 4: UNDERSTANDING OF SUDDEN CARDIAC ARREST SYMPTOMS AND WARNING SIGNS

PIAA SECTION 6: HEALTH HISTORY
To verify that all paperwork has been submitted in ATS, click on the “Immunizations/Paperwork” tab. On the left hand side is the list of forms, and there are columns that say “submitted” and “submitted date”. If you have correctly submitted, the forms there should be a “Yes” in the submitted column and a date in the “Submit Date” column. Once I have verified the paperwork there will be a “Yes” in the “Verified” column, and if all the paperwork is correct, there will be a checkmark in the “Paperwork Complete” box.
The PIAA requires that the physical for the 2021-2022 school year be completed no earlier than June 1 ,2021. When scheduling your athlete’s physical please take into consideration the possibility of long wait times to get an appointment. You can submit your online paperwork and physical any time after June 1st, regardless of what season you are participating in. Under the “eFiles” tab you will find PIAA Section 7. Please print it out and take it to your physician.
You can scan the physical and upload it directly into your student’s record via ATS. If you are having problems with that process, you can e-mail me the Section 7 and I will upload it for you. Due to COVID-19 we prefer these methods as opposed to turning in a piece of paper. There are several free apps for smartphones that have the ability to scan to a PDF file. This is the preferred file type for upload. If you have an iPhone there is a scan feature in the notes app, you can find a tutorial here: https://support.apple.com/en-us/HT210336
***Please note: many physicians wish to review Section 6 “Health History”, you should print a copy after you complete it to give to your physician at the physical. Also once you upload the form it will show up on the uploaded list but will not show as checked off under paperwork until someone visually verifies the page.***
Concussion Education and ImPACT testing:
Due to the success of the online testing last year the concussion education and ImPACT testing will be completed online from home. Once a sufficient number of athletes have registered for a team, I will be sending out instructions for completing the education portion and how to take the ImPACT test. This information will be sent to the athlete’s CRSD student e-mail address only.
If you have any problems please contact me at mhoran@crsd.org or 215-944-1368.
